Verbal Authorization Script – Text Messaging (SMS)
Thank you for agreeing to participate in this study. Cogstate would like to contact you by text message for administrative purposes, such as appointment reminders or scheduling notices. These text messages will not include medical information, details about your health, or information that identified you as being in a research study. Messages will be sent to the mobile phone number you provided.
Your phone number will only be accessible to the study coordinator and will be stored in a password protected database on a secure server. Contact information will be deleted when the study is over unless you have provided permission to be contacted again.
Receiving text messages is optional. You may choose not to receive texts, and this will not affect your participation. You may stop receiving text messages at any time by replying “STOP” or by contacting the study team. Standard messaging and data rates may apply.
